What is Oestrogel Pump-Pack used for?

Oestrogel is a topical HRT gel used in postmenopausal women. It contains estradiol, a form of oestrogen, and is prescribed to help manage the symptoms of menopause.

Oestrogel is used for:

  • Relief of menopausal symptoms – including hot flushes, night sweats, and other symptoms caused by falling oestrogen levels after the menopause
  • Prevention of osteoporosis – for postmenopausal women at increased risk of fractures where other treatments are not suitable

How does Oestrogel work?

During the menopause, your body produces less oestrogen. This drop in hormone levels is what causes symptoms like hot flushes, night sweats, and mood changes.

Oestrogel works by replacing the oestrogen your body is no longer making. The gel is applied to your arms or thighs, where estradiol is absorbed directly into the bloodstream. This raises your oestrogen levels back to a range similar to before the menopause, helping to ease symptoms.

Because Oestrogel is a transdermal gel, the hormone bypasses the digestive system and goes straight into the bloodstream through the skin.

Who can use Oestrogel?

Oestrogel is suitable for postmenopausal women whose symptoms are seriously affecting their daily life, or who are at increased risk of osteoporosis after menopause.

Do not use Oestrogel if you:

  • Are allergic to estradiol or any other ingredient in the gel
  • Have or have ever had breast cancer, or are suspected of having it
  • Have a cancer sensitive to oestrogens, such as cancer of the womb lining, or are suspected of having it
  • Have any unexplained vaginal bleeding
  • Have excessive thickening of the womb lining (endometrial hyperplasia) that is not being treated
  • Have or have ever had a blood clot in a vein (thrombosis), such as deep vein thrombosis or pulmonary embolism
  • Have a blood clotting disorder, such as protein C, protein S, or antithrombin deficiency
  • Have or recently had a disease caused by blood clots in the arteries, such as a heart attack, stroke, or angina
  • Have or have ever had a liver disease where liver function tests have not returned to normal
  • Have a rare inherited blood problem called porphyria

Speak to your doctor before using Oestrogel if you have ever had any of the following:

  • Fibroids or endometriosis
  • Increased risk of blood clots
  • High blood pressure
  • Diabetes
  • Gallstones
  • Migraine or severe headaches
  • Epilepsy or asthma
  • A liver disorder
  • Systemic lupus erythematosus (SLE)
  • Very high levels of fat in your blood (triglycerides)
  • Fluid retention due to heart or kidney problems

Your doctor will assess whether Oestrogel is suitable based on your medical history and any risk factors you may have.

How to Use Oestrogel & Dosage

Apply the gel once a day at the same time each day, either in the morning or in the evening. If you are unsure how many pumps to use, follow your doctor’s instructions – the dose of gel that has been prescribed will be the lowest amount needed to manage your symptoms.

If you are switching from another type of HRT, finish your current pack before starting Oestrogel.

Before using a new pump pack for the first time, prime it by pressing the plunger down a few times until the gel comes out, then discard that first dose of gel before you use it:

  1. Make sure your hands and the application site are clean, dry, and unbroken
  2. Press the plunger down firmly to dispense one measure of gel
  3. Apply to either the outer arms and shoulders, or the mid-inner thighs – spreading over a large area
  4. Wash your hands and leave them to dry for 5 minutes before dressing

Don’t apply your daily dose on or near the breasts or genital area. Avoid applying other skin products to the area for at least one hour after use.

Side Effects of Oestrogel Pump-Pack

Like all medicines, Oestrogel can cause side effects, though not everyone experiences them.

Common Side Effects

These may affect up to 1 in 10 people:

  • Headache
  • Nausea
  • Abdominal pain
  • Breast pain
  • Abnormal or irregular bleeding
  • Vaginal discharge
  • Weight change

Uncommon Side Effects

These may affect up to 1 in 100 people:

  • Depression or mood swings
  • Migraine
  • Vertigo
  • Vomiting
  • Vaginal yeast infection (thrush)
  • Inflamed vagina (vaginitis)
  • Feeling of weakness

Rare Side Effects

These may affect up to 1 in 1,000 people:

  • High blood pressure
  • Acne
  • Change in libido
  • Glucose intolerance
  • Bone pain

Other ingredients: Carbomer, trolamine, ethanol, purified water

Oestrogel Pump-Pack Warnings

  • Oestrogel is not a contraceptive. If it has been less than 12 months since your last period, or you are under 50, you may still need to use additional contraception
  • Using oestrogen-only HRT increases the risk of endometrial cancer. If you still have your womb, your doctor will prescribe a progestogen alongside Oestrogel
  • HRT use is associated with a slightly increased risk of breast cancer, ovarian cancer, blood clots, and stroke. Discuss the benefits and risks with your doctor before starting treatment and at least once a year while using it
  • Do not allow the gel to transfer to others, especially children. Cover the application area once dry and wash any exposed skin with soap and water immediately if contact happens
  • Oestrogel contains ethanol and is flammable. Avoid naked flames or sources of heat until the gel has fully dried on the skin
  • Some medicines can affect how Oestrogel works, including certain epilepsy medications, tuberculosis treatments, HIV medicines, and St John’s Wort. Always tell your doctor or pharmacist you are using Oestrogel if any new medicines are prescribed
  • If you are due to have surgery, tell your surgeon you are using Oestrogel. You may need to stop using it 4 to 6 weeks before the operation to reduce the risk of blood clots
  • Keep out of the sight and reach of children
  • Do not use after the expiry date shown on the packaging

Oestrogel replaces the oestrogen your body stops producing after the menopause. By absorbing estradiol through the skin, it raises your oestrogen levels back to a range similar to before the menopause. This helps relieve symptoms like hot flushes, night sweats, and mood changes, and can also help protect against bone loss (osteoporosis) in women at increased risk.

Apply Oestrogel to either the outer arms and shoulders, or the mid-inner thighs. Spread each pump over a large area of skin for the best absorption. Never apply it on or near the breasts or genital area.

There is no clinical difference between applying Oestrogel in the morning or evening. The most important thing is to apply it at the same time each day. Choose whichever time fits most easily into your routine and stick to it.

Avoid applying other skin products, cosmetics, or sunscreen to the same area for at least one hour after applying the gel. Don’t wash the application site for at least one hour, avoid close skin contact with your partner for one hour after application, and keep the area covered until the gel has dried to prevent accidental transfer – particularly to children.

At the standard dose of 2 pumps per day, each pump pack lasts 4 weeks. If you have been prescribed 4 pumps per day, it will last two weeks.

Most women notice an improvement in symptoms within a few weeks of starting treatment. However, it can take up to three months to feel the full benefit. If your symptoms haven’t improved after this time, speak to your doctor.

Yes, but not at the same time or on the same area. Wait at least one hour after applying Oestrogel before using moisturisers, sunscreens, or other skin products on the application site. Applying other products too soon can affect how well the gel is absorbed through the skin.

Weight change is listed as a common side effect of Oestrogel, affecting up to 1 in 10 people. However, some weight changes around the menopause are related to hormonal shifts rather than HRT itself. If you are concerned about changes in your weight while using Oestrogel, speak to your doctor.

If you miss a dose and your next dose is more than 12 hours away, apply it as soon as you remember. Otherwise, skip it and continue as normal. Never use 2 doses at once to make up for a missed one. Missing a dose may cause breakthrough bleeding or spotting.
