Mounjaro and Menopause: Can weight loss injections help with menopausal weight gain?

Weight gain during perimenopause and menopause is incredibly common. Many women find that their usual diet plan stops working, they gain weight around their middle, and exercise doesn’t have the same effect it used to.

Mounjaro (tirzepatide) has been making headlines as an effective weight loss medication, but can you use it during menopause? Will it mess with your hormones? And if you’re already on HRT, is it safe to take both?

What is Menopausal Weight Gain?

At least 50% of women gain weight during menopause. For most people, this happens gradually, putting on around 0.5kg per year during their 40s and 50s.

During menopause, falling oestrogen levels cause fat to redistribute from the hips and thighs to the abdomen. This is why so many women notice their waistline expanding even if they haven’t gained much weight overall.

There are several reasons why women gain weight during the menopause:

Does Mounjaro affect hormones?

Weight loss typically improves how your body responds to insulin. This can help stabilise your overall hormonal balance, particularly if you had insulin resistance or metabolic issues before starting treatment.

Changes in Hormone Absorption

Mounjaro slows down gastric emptying (the rate at which food leaves your stomach), which can reduce the absorption of oral medications, including oral HRT, ​​potentially making them less effective or delaying how quickly they work in the body.

How can Mounjaro help with menopause weight gain?

Mounjaro for weight loss addresses several of the mechanisms that make weight loss so difficult during menopause.

A study presented at ENDO 2025 looked at 120 postmenopausal women taking tirzepatide over 18 months. The results were:

  • Women taking HRT alongside Mounjaro lost 17% of their body weight
  • Women taking Mounjaro without HRT lost 14% of their body weight
  • 45% of women on HRT achieved at least 20% weight loss, compared to just 18% without HRT

The researchers noted this was the first evidence showing that combining tirzepatide with HRT significantly boosts treatment effectiveness in postmenopausal women.

This medication supports weight loss in multiple ways.

  • It reduces appetite: This makes it easier to eat less without feeling constantly hungry, which is particularly helpful when your metabolism has slowed down.
  • It improves insulin sensitivity: Mounjaro improves how your body responds to insulin, helping reverse this metabolic shift.
  • It slows gastric emptying: You’ll feel fuller for longer, which helps you naturally reduce portion sizes and snack less between meals.
  • It targets abdominal fat: While Mounjaro produces overall weight loss, many users report losing weight from their midsection – exactly where menopausal weight tends to accumulate.

Mounjaro & HRT: Can I use them at the same time?

Can Mounjaro affect my oral HRT medication?

Because Mounjaro causes slower gastric emptying, food (and medications) stay in your stomach longer. This delays and reduces the absorption of oral medications (including oral HRT).

Studies on oral contraceptives found that tirzepatide reduced peak absorption levels by up to 66%. While specific studies on oral HRT haven’t been published, the same mechanism applies.

If you’re taking oral HRT (tablets containing oestrogen or progesterone), like Gepretix, Progynova or Utrogestan,  you have three options:

  • Switch to transdermal HRT: Patches, gels, sprays bypass your digestive system entirely. The hormones absorb directly through your skin, so Mounjaro won’t affect them.
  • Use a hormonal IUD: Devices like the Mirena coil release progesterone locally and aren’t affected by gastric emptying.
  • Temporarily increase your oral dose: If you need to stay on oral HRT, your doctor may increase the dose for the first four weeks after starting Mounjaro or after each dose increase. This compensates for reduced absorption.

Can Mounjaro help with a hormonal imbalance?

Mounjaro doesn’t directly treat hormonal imbalances. It doesn’t alter your oestrogen, progesterone, or other reproductive hormone levels. However, the weight loss and improved insulin sensitivity from Mounjaro can help stabilise your overall metabolic health, which may indirectly improve hormonal balance – particularly if you had insulin resistance or PCOS-related hormonal issues. Mounjaro cannot trigger menopause. Menopause occurs when your ovaries stop producing eggs and oestrogen levels decline naturally, typically between the ages of 45 and 55. Mounjaro works on completely different metabolic pathways and has no effect on ovarian function or the timing of menopause.

Can Mounjaro delay menopause?

Mounjaro cannot delay menopause. The timing of menopause is determined by your genetics and ovarian reserve – the number of eggs remaining in your ovaries. While significant weight loss can occasionally affect menstrual cycles in pre-menopausal women, it doesn’t extend your reproductive years or delay the natural onset of menopause.

Does Mounjaro increase oestrogen?

Mounjaro doesn’t increase oestrogen levels as the medication doesn’t influence your ovaries or hormone production. In fact, significant weight loss from Mounjaro may slightly reduce peripheral oestrogen production, since fat tissue naturally produces oestrogen through aromatisation. However, this reduction is minimal and not clinically significant for postmenopausal women, whose oestrogen levels are already low.

Can Mounjaro cause post-menopausal bleeding?

There is no established link between Mounjaro and post-menopausal bleeding. However, if you experience any vaginal bleeding 12 months or more after your last period, you must see your doctor urgently – regardless of whether you’re taking Mounjaro.

Post-menopausal bleeding is benign in around 85-90% of cases (often caused by vaginal atrophy), but it occasionally indicates serious conditions, including endometrial cancer. Never assume bleeding is caused by your medication without proper medical investigation.

Does Mounjaro cause hot flashes?

Hot flashes aren’t a recognised side effect of Mounjaro or any other weight loss drugs. Some women have reported flushing or hot sensations while taking tirzepatide, but this is quite rare and may coincide with natural menopause progression rather than being caused by the medication.
