How does Doxycycline work to treat chlamydia?

Doxycycline is an antibiotic and the first-line treatment for chlamydia that stops the bacteria from multiplying in your body.

Chlamydia is caused by bacteria called Chlamydia trachomatis. Doxycycline works by preventing these bacteria from making the proteins they need to survive and reproduce. Without these essential proteins, the bacteria die off, and your immune system can clear the infection.

When taken correctly, doxycycline is over 95% effective at clearing chlamydia infections.

What is Doxycycline for chlamydia used for?

Doxycycline 100mg is used to treat urogenital chlamydia, one of the most common sexually transmitted infections (STIs) in the UK. Chlamydia often has no symptoms, which is why many people don’t know they have it.

When symptoms do appear, they can include:

  • Unusual discharge from the vagina, penis, or rectum
  • Pain when urinating
  • Pain during sex
  • Bleeding between periods or after sex (in women)
  • Pain in the testicles (in men)
  • Pain or discharge from the rectum

Untreated chlamydia can cause serious health problems. In women, it can cause pelvic inflammatory disease (PID), which may lead to infertility or ectopic pregnancy. In men, it can cause testicular inflammation and may affect fertility. Chlamydia cannot resolve on its own and requires antibiotic treatment.

Doxycycline clears the infection and prevents these complications from developing. It’s important to get a chlamydia test and start treatment as soon as possible if you think you might have the infection.

Who can use Doxycycline for chlamydia?

Doxycycline is suitable for most adults who have had a positive chlamydia test and need antibiotic treatment.

However, some people cannot take doxycycline. You should not use doxycycline if you:

  • Are allergic to doxycycline, any other tetracycline antibiotic, or any of the ingredients in the capsules
  • Are pregnant or trying to become pregnant
  • Are breastfeeding
  • Are under 12 years old (doxycycline can affect tooth development in children)

If you cannot take doxycycline, your doctor can prescribe an alternative treatment such as azithromycin.

Before taking doxycycline, speak to your doctor or pharmacist if you:

  • Have kidney or liver problems
  • Have myasthenia gravis (a condition that causes muscle weakness)
  • Have systemic lupus erythematosus (SLE), an autoimmune condition
  • Have porphyria (a rare blood disorder)
  • Are likely to be exposed to strong sunlight or UV light
  • Have problems with your stomach or intestines, or often get diarrhoea when taking antibiotics
  • Are taking oral retinoids for acne
  • Have inflammation of the oesophagus (gullet)

How to Take Doxycycline 100mg for Chlamydia Treatment

For treating chlamydia with doxycycline, the usual dose is 100mg twice daily for 7 days.

  1. Take one 100mg capsule twice a day – once in the morning and once in the evening.
  2. Swallow the capsule whole with plenty of water while sitting or standing upright.
  3. Take your capsules at roughly the same times each day to maintain steady levels in your body.
  4. You can take doxycycline with or without food. If it upsets your stomach, taking it with food or milk may help.
  5. Do not lie down for at least 30 minutes after taking the capsule. This helps prevent irritation of your throat or oesophagus.

Complete the full course of doxycycline, even if your symptoms disappear before you finish. Stopping early can mean the infection isn’t fully cleared.

Avoid taking antacids, iron supplements, or zinc at the same time as doxycycline, as these can interfere with the antibiotic’s effectiveness. Leave at least 2 hours between taking these products and your doxycycline capsule.

You should avoid having sex (including oral sex) until you and your partner have both finished treatment and been given the all-clear. This usually means waiting 7 days after completing your course of antibiotics.

Side Effects of Doxycycline

Like all medicines, doxycycline can cause side effects, although not everyone experiences them. Most side effects are mild and often settle down as your body adjusts to the medicine.

  • Feeling or being sick
  • Headache

Uncommon Side Effects

These may affect up to 1 in 100 people:

  • Heartburn
  • Vaginal infection (such as thrush)

Rare Side Effects

These may affect up to 1 in 1,000 people:

  • Anxiety
  • Difficulty swallowing, sore or painful tongue or mouth
  • Skin reddening (flushing)
  • Ringing or buzzing noise in the ear
  • Soreness and itching of the genital area
  • Inflammation of the bowel
  • Skin sensitivity to sunlight (you may burn more easily)
  • Inflammation and damage to the liver
  • Abnormal liver function tests
  • Yellowing of skin and eyes (jaundice), inflammation of the pancreas
  • Upset stomach, loss of appetite, diarrhoea, stomach pain
  • Darker patches on your skin

Serious Side Effects

Stop taking doxycycline and contact your doctor immediately or go to A&E if you experience:

  • Sudden difficulty breathing, chest pain, swelling of face or lips, severe rash or itching (signs of an allergic reaction)
  • Severe, persistent, or bloody diarrhoea (this can occur up to 3 months after treatment and may be a sign of serious bowel inflammation)
  • Very severe headache with visual disturbances such as blurred vision, double vision, or loss of vision
  • Serious widespread blistering of the skin, mouth, eyes, or genitals
  • Increased pressure in the brain
  • Worsening symptoms of systemic lupus erythematosus (joint pain, skin rash, fever)

If you experience skin that is more sensitive to sunlight than normal, with rash, itching, redness, or severe sunburn, stop using doxycycline and tell your doctor.

You can report side effects to the MHRA (Medicines and Healthcare Products Regulatory Agency) through the Yellow Card Scheme to help monitor the safety of the medication.

Doxycycline Warnings

  • Do not use doxycycline if you are pregnant, trying to become pregnant, or breastfeeding. It can affect tooth development and bone growth in unborn babies and infants.
  • Avoid exposure to strong sunlight, sunbeds, and UV lamps while taking doxycycline. Your skin will be more sensitive to sunburn. Use high-factor sunscreen (SPF 30 or higher) and wear protective clothing if you need to be in the sun.
  • Alcohol may reduce the effectiveness of doxycycline and should be avoided during treatment.
  • Stop using doxycycline and contact your doctor immediately if you develop severe or persistent diarrhoea, bloody stools, or severe stomach pain, as these could be signs of a serious bowel condition.
  • If you need to have surgery or dental procedures requiring anaesthetic while taking doxycycline, tell your doctor or dentist.
  • Keep doxycycline out of sight and reach of children.
  • Store below 25°C in the original package in a dry place.
  • Do not use after the expiry date shown on the packaging.

Some people prefer azithromycin for convenience, while doxycycline may be better if you struggle to remember single-dose medications or have certain medical conditions.

It’s best to avoid alcohol while taking doxycycline. Alcohol can reduce the effectiveness of the antibiotic and may increase side effects like upset stomach and nausea. Wait until you’ve finished your course before drinking.
