What is Imvaggis used for?

Imvaggis pessaries are used to treat vaginal atrophy and dryness caused by menopause. Imvaggis is a form of vaginal hormone replacement therapy (HRT) that restores oestrogen locally in the vaginal tissues. As oestrogen levels fall after menopause, the vaginal tissues can become thinner, drier and more sensitive.

Symptoms of vaginal atrophy include:

  • Vaginal dryness
  • Itching and irritation
  • Burning or soreness
  • Discomfort or pain during sex
  • Discomfort when urinating

How do Imvaggis pessaries work?

Imvaggis contains estriol, a naturally occurring form of oestrogen. After menopause, falling oestrogen levels cause the vaginal tissues to thin and dry out. Imvaggis works by delivering a low dose of oestrogen directly to the vaginal area, helping to restore moisture and improve the condition of the vaginal tissues.

Because the pessary is inserted directly into the vagina, the hormone is released exactly where it is needed. Absorption into the bloodstream is very low, which means the risks associated with systemic HRT are less likely to apply.

Who can use Imvaggis pessaries?

Imvaggis is suitable for postmenopausal women experiencing vaginal symptoms caused by oestrogen deficiency.

Do not use Imvaggis if you:

  • Are allergic to estriol or any of the other ingredients
  • Have or have had breast cancer, or are suspected of having it
  • Have or suspect you have a cancer that is sensitive to oestrogens, such as cancer of the womb lining (endometrial cancer)
  • Have any unexplained vaginal bleeding
  • Have excessive thickening of the womb lining (endometrial hyperplasia) that is not being treated
  • Have or have ever had a blood clot in a vein (thrombosis), such as in the legs or lungs
  • Have a blood clotting disorder (such as protein C, protein S or antithrombin deficiency)
  • Have or recently had a disease caused by blood clots in the arteries, such as a heart attack, stroke or angina
  • Have or have ever had a liver disease where your liver function tests haven’t returned to normal
  • Have a rare inherited blood problem called porphyria

Speak to your doctor before using Imvaggis if you have ever had any of the following, as these may return or worsen during treatment:

  • Fibroids inside your womb
  • Endometriosis or a history of excessive growth of the womb lining
  • Increased risk of blood clots
  • Increased risk of an oestrogen-sensitive cancer
  • High blood pressure
  • A liver disorder
  • Diabetes
  • Gallstones
  • Migraine or severe headaches
  • Lupus (SLE)
  • Asthma
  • Epilepsy
  • A disease affecting the eardrum and hearing (otosclerosis)
  • Very high levels of fat in your blood (triglycerides)
  • Fluid retention due to heart or kidney problems
  • Hereditary or acquired angioedema

How to Use Imvaggis Pessaries

Always use Imvaggis exactly as your doctor has told you. If you are unsure, check with your doctor or pharmacist.

The pessary should be inserted deeply into the vagina, preferably in the evening before going to bed. To administer a pessary, tear the foil apart until the pessary can be removed easily.

Initial dose: One pessary daily for the first 3 weeks of treatment.

Maintenance dose: After the first 3 weeks, one pessary twice a week.

Your doctor will aim to prescribe the lowest effective dose for the shortest duration needed to manage your symptoms.

Side Effects of Imvaggis Pessaries

Like all medicines, Imvaggis can cause side effects, though not everyone experiences them. Because Imvaggis is a local treatment with very low absorption into the bloodstream, side effects are generally less common than in women taking HRT systemically.

Common Side Effects

May affect up to 1 in 10 users:

  • Vulvovaginal burning, itching and pain
  • Discomfort when urinating (dysuria)

Uncommon Side Effects

May affect up to 1 in 100 users:

  • Vaginal discharge
  • Anorectal discomfort

Imvaggis Pessaries Ingredients

Active ingredient: 0.03 mg estriol per pessary

Other ingredients: Butylhydroxytoluene, glycerol mono/bis [(Z-R)-12-hydroxyoctadec-9-enoate], hard fat, macrogol cetostearyl ether

Imvaggis Warnings and Precautions

  • Imvaggis isn’t a contraceptive. If it is less than 12 months since your last period, or you are under 50, you may still need additional contraception to prevent pregnancy
  • Any vaginal infections should be treated before starting Imvaggis
  • Tell your surgeon you are using Imvaggis if you are due to have an operation. You may need to stop using it 4 to 6 weeks beforehand to reduce the risk of a blood clot
  • If you experience any unexplained vaginal bleeding or spotting, contact your doctor
  • Regularly check your breasts and attend mammography screening as recommended by your doctor
  • If you notice yellowing of your skin or the whites of your eyes, stop using Imvaggis and see a doctor immediately
  • If you experience signs of a blood clot, such as painful swelling and redness of the legs, sudden chest pain or difficulty breathing, stop using Imvaggis and seek urgent medical attention
  • If you develop a severe headache or migraine for the first time, stop using Imvaggis and contact your doctor
  • Keep out of the sight and reach of children
  • Do not use after the expiry date shown on the packaging
  • Store below 25°C
  • Butylhydroxytoluene (an ingredient in Imvaggis) may cause local skin reactions or irritation to the eyes and mucous membranes
  • If you use latex condoms at the same time as Imvaggis, be aware that Imvaggis may reduce the strength of latex and affect the reliability of the condom

Imvaggis is for postmenopausal women only, so it is intended for use after your periods have stopped. If you are still having periods or are unsure whether you have reached menopause, speak to your doctor before starting treatment.

Some women may notice light bleeding or spotting during treatment. This is usually nothing to worry about, but you should make an appointment to see your doctor if it happens, as it can occasionally be a sign that the womb lining has become thicker. Any unexplained vaginal bleeding should always be discussed with your doctor.

Imvaggis and Vagifem contain different active ingredients. Imvaggis contains estriol (0.03mg), while Vagifem contains estradiol (10 micrograms). Both are local vaginal oestrogen treatments used for vaginal atrophy symptoms. They aren’t directly comparable in terms of strength as they use different oestrogens. Your doctor or pharmacist can advise which is most appropriate for you.

Any existing vaginal infections should be treated and cleared with appropriate medicines before you start using Imvaggis. Speak to your doctor or pharmacist if you think you have an infection before beginning treatment.

Don’t throw Imvaggis pessaries away via wastewater or household waste. Ask your pharmacist how to dispose of any medicines you no longer need. This helps protect the environment.

Most women notice improvement in their symptoms within a few weeks of starting treatment. However, it may take longer to experience the full benefit. Continue using Imvaggis as prescribed even if you don’t notice immediate results, and speak to your doctor if your symptoms haven’t improved after three months of using Imvaggis estriol.

During the first 3 weeks (daily use): if you don’t realise you have missed it until the next day, skip the missed dose and continue your usual schedule.

During the maintenance phase (twice-weekly use): take the missed dose as soon as possible.

If you use more pessaries than you should, there is no need to panic, but you should speak to your doctor for advice. You may feel or be sick, and some women experience vaginal bleeding after a few days.
