Imvaggis pessaries are used to treat vaginal atrophy and dryness caused by menopause. Imvaggis is a form of vaginal hormone replacement therapy (HRT) that restores oestrogen locally in the vaginal tissues. As oestrogen levels fall after menopause, the vaginal tissues can become thinner, drier and more sensitive.
Symptoms of vaginal atrophy include:
Imvaggis contains estriol, a naturally occurring form of oestrogen. After menopause, falling oestrogen levels cause the vaginal tissues to thin and dry out. Imvaggis works by delivering a low dose of oestrogen directly to the vaginal area, helping to restore moisture and improve the condition of the vaginal tissues.
Because the pessary is inserted directly into the vagina, the hormone is released exactly where it is needed. Absorption into the bloodstream is very low, which means the risks associated with systemic HRT are less likely to apply.
Imvaggis is suitable for postmenopausal women experiencing vaginal symptoms caused by oestrogen deficiency.

Always use Imvaggis exactly as your doctor has told you. If you are unsure, check with your doctor or pharmacist.
The pessary should be inserted deeply into the vagina, preferably in the evening before going to bed. To administer a pessary, tear the foil apart until the pessary can be removed easily.
Initial dose: One pessary daily for the first 3 weeks of treatment.
Maintenance dose: After the first 3 weeks, one pessary twice a week.
Your doctor will aim to prescribe the lowest effective dose for the shortest duration needed to manage your symptoms.
Like all medicines, Imvaggis can cause side effects, though not everyone experiences them. Because Imvaggis is a local treatment with very low absorption into the bloodstream, side effects are generally less common than in women taking HRT systemically.

Weight gain isn’t listed as a side effect of Imvaggis in the patient information leaflet. Because Imvaggis is a local treatment with very low absorption into the bloodstream, it is unlikely to cause the systemic effects sometimes seen in women who use HRT in tablet or patch form. If you have any concerns, speak to your doctor or pharmacist.
Imvaggis is for postmenopausal women only, so it is intended for use after your periods have stopped. If you are still having periods or are unsure whether you have reached menopause, speak to your doctor before starting treatment.
Some women may notice light bleeding or spotting during treatment. This is usually nothing to worry about, but you should make an appointment to see your doctor if it happens, as it can occasionally be a sign that the womb lining has become thicker. Any unexplained vaginal bleeding should always be discussed with your doctor.
Imvaggis and Vagifem contain different active ingredients. Imvaggis contains estriol (0.03mg), while Vagifem contains estradiol (10 micrograms). Both are local vaginal oestrogen treatments used for vaginal atrophy symptoms. They aren’t directly comparable in terms of strength as they use different oestrogens. Your doctor or pharmacist can advise which is most appropriate for you.
Any existing vaginal infections should be treated and cleared with appropriate medicines before you start using Imvaggis. Speak to your doctor or pharmacist if you think you have an infection before beginning treatment.
Don’t throw Imvaggis pessaries away via wastewater or household waste. Ask your pharmacist how to dispose of any medicines you no longer need. This helps protect the environment.
Most women notice improvement in their symptoms within a few weeks of starting treatment. However, it may take longer to experience the full benefit. Continue using Imvaggis as prescribed even if you don’t notice immediate results, and speak to your doctor if your symptoms haven’t improved after three months of using Imvaggis estriol.
During the first 3 weeks (daily use): if you don’t realise you have missed it until the next day, skip the missed dose and continue your usual schedule.
During the maintenance phase (twice-weekly use): take the missed dose as soon as possible.
If you use more pessaries than you should, there is no need to panic, but you should speak to your doctor for advice. You may feel or be sick, and some women experience vaginal bleeding after a few days.
