What is Sandrena gel used for?

Sandrena gel is a hormone replacement therapy (HRT) used to relieve menopause symptoms. During the menopause, oestrogen levels in the body fall, which can trigger a range of symptoms that affect daily life.

Sandrena is prescribed to treat:

  • Hot flushes and night sweats
  • Mood changes and irritability
  • Sleep disturbances
  • Vaginal dryness and discomfort
  • Joint and muscle aches

Sandrena can also be prescribed to help prevent osteoporosis (fragile bones) after the menopause, particularly in women who are at an increased risk of fractures and where other treatments aren’t suitable.

How does Sandrena gel work?

Sandrena is a transdermal oestrogen gel containing estradiol, a form of oestrogen identical to that naturally produced by the body. When applied to the skin, estradiol is absorbed into the bloodstream, helping restore oestrogen levels.

By topping up falling oestrogen levels, Sandrena helps to reduce the frequency and severity of symptoms of menopause such as hot flushes, night sweats, and mood changes. Because the gel is applied directly to the skin rather than swallowed, it bypasses the digestive system and goes straight into the bloodstream.

Who can use Sandrena gel?

Sandrena gel is suitable for postmenopausal women experiencing symptoms caused by falling oestrogen levels. It is a prescription-only medicine, and your doctor will discuss the benefits and risks with you before you take HRT to ensure it is the right option for you.

Do not use Sandrena if you:

  • Have or have ever had breast cancer, or are suspected of having it
  • Have cancer that is sensitive to oestrogens, such as cancer of the womb lining (endometrial cancer), or are suspected of having it
  • Have any unexplained vaginal bleeding
  • Have excessive thickening of the womb lining (endometrial hyperplasia) that is not being treated
  • Have or have ever had a blood clot in a vein (thrombosis), such as in the legs (deep vein thrombosis) or the lungs (pulmonary embolism)
  • Have a blood clotting disorder, such as protein C, protein S, or antithrombin deficiency
  • Have or recently have had a disease caused by blood clots in the arteries, such as a heart attack, stroke, or angina
  • Have or have ever had a liver disease where your liver function tests have not returned to normal
  • Have a rare inherited blood problem called porphyria
  • Are allergic to estradiol or any of the other ingredients in Sandrena

Women with a history of any of the following conditions should speak to their doctor before using Sandrena, as these may return or worsen during treatment:

  • Fibroids inside your womb
  • Endometriosis or a history of excessive growth of the womb lining
  • An increased risk of blood clots
  • An increased risk of oestrogen-sensitive cancer (for example, if a close family member has had breast cancer)
  • High blood pressure
  • A liver disorder, such as a benign liver tumour
  • Diabetes
  • Gallstones
  • Migraine or severe headaches
  • Systemic lupus erythematosus (SLE)
  • Epilepsy
  • Asthma
  • A disease affecting the eardrum and hearing (otosclerosis)
  • Very high levels of fat in the blood (triglycerides)
  • Fluid retention due to heart or kidney problems
  • Hereditary or acquired angioedema

How to Use Sandrena Gel

Sandrena gel is applied once a day to the skin on your lower body or thighs. Always follow the instructions given by your doctor or pharmacist.

  1. Choose a clean, dry area of skin on your lower body or thighs
  2. Apply the gel and spread it over an area roughly one to two times the size of your hand
  3. Alternate sides each day – apply to the left side one day and the right side the next
  4. Allow the gel to dry for a few minutes before covering with clothing
  5. Wash your hands thoroughly with soap and water after applying

Do not wash the application area for at least one hour after use.

Do not apply the gel to your breasts, face, or any irritated skin.

Do not allow others to touch the application area until the gel has fully dried.

Sandrena Gel Dose

Sandrena is available in 0.5mg and 1mg sachets. The recommended dose is 0.5-1.5mg of estradiol per day, with 1.5mg as the maximum. Your doctor will prescribe the lowest dose needed to manage your symptoms.

  • 0.5mg daily: use one 0.5g sachet
  • 1mg daily: use one 1mg sachet, or two 0.5g sachets
  • 1.5mg daily: use three 0.5g sachets, or one 0.5g sachet and one 1g sachet

Sandrena is an oestrogen-only HRT. If you still have your womb, your doctor will usually also prescribe a progestagen tablet to take for 12 to 14 days of each monthly cycle. This protects the lining of the womb. After each course of progestagen, you will normally have a withdrawal bleed.

Sandrena Gel Side Effects

Like all medicines, Sandrena gel can cause side effects, although not everyone experiences them. Some side effects, such as breakthrough bleeding and breast tenderness, are more common during the first few months of treatment and often settle with continued use.

Common Side Effects

These may affect up to 1 in 10 people:

  • Skin irritation, itching, or rash at the application site
  • Breast tenderness or pain
  • Headache or dizziness
  • Stomach pain, nausea, vomiting, or flatulence
  • Breakthrough bleeding or spotting
  • Swollen feet and lower legs
  • Hot flushes
  • Depression, nervousness, or lethargy
  • Weight changes

Uncommon Side Effects

These may affect up to 1 in 100 people:

  • Migraine
  • Changes to sex drive or mood, anxiety, or difficulty sleeping
  • Visual disturbances or dry eyes
  • High blood pressure
  • Acne, hair loss, or dry skin
  • Joint pain or muscle cramps
  • Urinary tract infection or increased urinary frequency
  • Increased heart rate
  • Allergic reaction

Rare Side Effects

These may affect up to 1 in 1,000 people:

  • Blood clots in a vein (venous thromboembolism)
  • Changes in liver function
  • Menstrual pain

Stop using Sandrena and seek medical attention straight away if you notice a sudden rise in blood pressure, yellowing of the skin or whites of the eyes, sudden migraine-type headaches, or any signs of a blood clot such as painful swelling of the legs, sudden chest pain, or difficulty breathing.

Important Information

  • Keep out of the sight and reach of children
  • Do not use after the expiry date shown on the carton and sachet
  • Store below 25°C
  • Sandrena is not a contraceptive. If it is less than 12 months since your last period, or you are under 50, you may still need to use additional contraception to prevent pregnancy
  • Do not use Sandrena if you are pregnant or breastfeeding
  • The gel can transfer to other people through skin contact. Wash your hands after applying, cover the area with clothing once the gel has dried, and shower the application site before close skin contact with others
  • Keep children away from the application area. If a child comes into contact with the gel, wash their skin with soap and water immediately and contact a healthcare provider if you notice any signs of unexpected puberty
  • If you need a blood test, tell your doctor or the laboratory that you are using Sandrena, as it can affect the results of some tests
  • Some medicines can interfere with how Sandrena works, including certain epilepsy medicines, tuberculosis treatments, HIV medicines, and St John’s Wort. Always tell your doctor or pharmacist about any other medicines you are taking

Sandrena and Oestrogel both contain the same active ingredient, estradiol, but they are dosed differently. Lethargy is listed as a common side effect of Sandrena gel, affecting up to 1 in 10 people. If you find that tiredness is affecting your daily life during treatment, mention it to your doctor, as your dose may need to be reviewed.

Hair loss (alopecia) is listed as an uncommon side effect of Sandrena gel, meaning it may affect up to 1 in 100 people. If you notice unusual hair loss during treatment, speak to your doctor.

Sandrena gel should only be applied to the skin on your lower body or thighs. Do not apply it to your breasts, face, or any irritated skin. Applying it to other areas is not recommended and may affect how the gel is absorbed.

If you forget to apply Sandrena gel, apply it as soon as you remember – unless you are more than 12 hours late. If more than 12 hours have passed, skip the missed dose and continue as normal the following day. Do not apply a double dose to make up for a missed one. Missing doses may cause some breakthrough bleeding.
